Incarnation mod modifies the ia of some enemies and bosses and add new ones, it isn’t difficult as resurrection and lmtsr but it's like an improvement of the vanilla game.
Resurrection is difficult the first time you play it add new bosses and modify the attack pattern of the main bosses, i recommend it when you finish a charmless run, because some bosses especially saint Isshin and owl father have some crazy combos.
lmtsr it's very difficult and sometimes unfair, adds a lot of objects, skills prosthetics and 40 new minibosses the difficult of this mods it can be compared to ng++ charmless + db run because some bosses applies a new statues effect called curse that is worse than terror, but you should give it a try, this mod is like a new game.
If you want to play a moded version of sekiro i recommend to start a new playthrought, the only mods that don't modify the vanilla game are incarnation and randomizer, resurrection and lmtsr adds new objects and skills that aren't in the base game.
The vanilla game is accessible the only thing is that you have to eliminate or moved modengine out of your sekiro game folder

As an exploration enthusiast, I'll try this first.
LMTSR with the charm next then, since it is the real game changer.
I have done all sekiro (including playtrough 8 charmless/bell) with a malfunctioning gamepad. I'll remain into familiar waters in terms of difficulty and unfairness.
I'll probabely have to go until ng+2 on LMTSR to get all the old/new skills and prosthethics ...
lmtsr though is brutally hard and don't really recommend it unless you are extremely good at the game. heck, i can 100% speedrun the game in few hours without dying once but lmtsr is just too much at times.
resurrection I beat every boss except secret hirata owl dad and sword saint isshin. although i got close to killing him I can't say the same for owl dad. all the other bosses are harder too but not too hard. it's a really good mod, great fun.

That's why I was hestitating to begin with between LMTSR and Resurection.
I want new items/content/lore/quests/exploration, but I do not want a brutal challenge.
And I do not care about same bosses/ennemies, but with different combos.
I'll probabely end up trying LMTSR (after randomizer/incarnation), then reverting back to Resurection if it is unbearable. I already know the challenge is more about unfair/bad design choices ...
If only they have done LMTSR with the vanilla game difficulty, as an expansion rather than a punishment ... I wouldn't hesitate at all
The main problem is that this mod doesn't add much stuff. The small amount of encounters were changed poorly therefore nothing would've changed if there weren't these changes (sorry for tautology).
As for bosses, not all of them have been changed. I can just list changed bosses I've liked:
Chained ogre
Ganichiro
Seven Ashina Spears - Shikibu Toshikatsu Yamauchi
Armoured Warrior
Corrupted Monk.
Bosses, modders've added, are really garbage, tbh - it's just mobs with 2 lifes - lame af. Also I hate Snake Eyes Shirafuji rework.
In general, the bosses are changed ofc, but it feels like a normal walkthrough with no items changed and I doubt that it will be much better in the second half of the game.
Only faced Gyobu and existing minibosses (ogre, bull, samurais, 7spears...)
Even with the charm, it is way harder than ng+7 charmless+bell, and very unbalanced.
* regular trash mobs taking up to 7 hits to fill their posture bar
* Purple ninjas with unlimited combos and camera issues when they teleport behind you
* annoying swarms of mobs everywhere, making stealth a headache
* status effects feast
The mod puts too much emphasis on deflect instead of attack, I feel like fighting swordsmen with a bamboo stick.
In fact, they tempered so much with the calculation engine, you cannot finish Hanbei's full deflect tutorial without an ako sugar. Whithout it, Hanbei's posture doesn't reach the max after a fully deflected combo, and it insta resets.
If Sekiro was a great sandwich, LMTSR would be a heart attack burger. Not for relaxation.
Yes it is an ongoing work.
I hope the final result won't look like LMTSR, all over the place.
