I was wrong.
It does start in HDR even with it off in Windows and off in the game.
Toggling off/on in game doesn't actually turn it off.
Weird.
The off toggle in game is just making the game not recognize HDR is on instead of actually turning it back off.
Similar to having HDR on in a game that doesn't support it.
I guess it defaults to on until it applies your specific settings.
I verified by leaving my HDR TV info screen up to monitor input signal.
For me, I like HDR on,
However, the HDR in this particular game looks like poo. Washed out.
I will keep off too and use "Special K" to get HDR looking better.
Run in windowed mode instead of fullscreen.
It auto starts with HDR ON because game is set to Fullscreen.
I remembered that HDR can't be on in Windowed mode. This is true for many games.
I think it's the type of HDR used but not sure.
Anyway, running game in windowed mode, it can't run HDR and defaults to HDR off.
Furthermore, HDR will be greyed out in the menu since it can't run.
At this point, it can't even be selected.
If you don't like bordered window,
All you need to do is use external program or autohotkey script to remove border essentially making game borderless fullscreen.
Special K program can remove border too.

F12 is hotkey to toggle bordered/borderless fullscreen.
Injustice 2 is another game that if set to fullscreen mode, it will auto start with HDR ON.
So Sekiro isn't the only game with this odd behavior.
