Without the Charm, enemies do more Vitality Damage, Posture Damage, have more Vitality themselves and have higher Posture meters.
As for Genichiro's Floating Passage attack, the 5th or 6th hit has a different deflect timing window compared to his previous hits so button-mashing doesn't work. Most players either just wait it out or deflect the first 4 hits and dash away.
I'm not sure about the last question but if I had to guess, I don't think so. It would be funny though.

Second if you beat ng you can beat charmless+bell run, the game doesn't get harder, the combat is simply too shallow

across all of my 100 hours ish gameplay i manage to successfully deflect genichiro's full spiral cloud passage just "once" the rest is either dodge after 4th attack or umbrella
You can still take the last big slice from that combo tho by approaching him again. The tomoe form will give you a thrust for that which you can use for mikiri and big posture damage.

I gave up to parry the combo perfect.

Maybe the vid in general is helpfull. You see were you can hit him safe like how to exploit isshins ashina cross attack or his spear roundhouse slash.
Isshin is just pure parry focus. The window for parry is quite big although it doesnt seem like it on your first charmless run.
Its just pure training and endurance.
I don't remember the whole game but I am pretty sure isshin you can to bait the lunge attack but I think I still remember all the cheesing strats, which are usually just running around.
Btw I beat the whole game during my blind run without using tools, with bell and without cheesing anything but the drunkard,
Then when I went in ng+ with bell+charmless I didnt see the game become harder
edit: I even forgot you got all those op buff if you kill the headless
