Prosthetic upgrade issue

So, I just bought my first prosthetic upgrade, the spinning shuriken thing, but when I try to use it it just doesn't work. Like, at all. I went and grabbed the first axe upgrade too, but holding RT doesn't do anything. Has anyone else had this issue? Any idea what I might be doing wrong?

u have to switch to the upgraded version in your inventory...

Did you actually equip it? I forgot that too the first time which got me killed against that first mini boss^^

As a matter of fact, I did not. Didn't know you had to do that, thought it was an automatic upgrade. Thanks, yo.

the first upgrades are but not all future ups are. Some change the function of the tool quite a lot. So the game solves this by making EVERY upgrade available as separate items even when its redundant.
