Someone explain the dragon rot mechanics please.
And how much should I care about it? Thanks.

NPCs do not die of dragonrot.

dragonrot only progresses if you have to respawn at a checkpoint - Using resurrection charges does not do anything.

dragonrot applications are random (or appear to be).

dragonrot only affects NPCs you've talked to AFAIK.

unseen aid does not prevent dragonrot.

endings do not appear to be affected by dragonrot.

There are a limited number of dragonrot-curing items, but one item will cure all currently afflicted NPCs.

that should cover it.

Dragonrot causes you to have a lower chance for Unseen Aid (Not losing anything on Death). It also prevents you from being able to progress any character related quests if that character is afflicted. This can mean being locked out of the ability to upgrade a merchant's shop through their questline, for example.

The more you die, the more likely someone will contract Dragonrot.

It’s to punish you and make you feel bad about yourself

When you die there is a certain chance that one of your found and met NPC (I've never recieved ROT on an NPC I hadn't met yet) to get ROT. The only thing this does is prevent quest progress. Your NPC will not die. You can use a Dragon's Blood Droplet to cure ALL currently afflicted NPC's. Early on this mechanic bothered me a lot since I did not know how available these Dragon's Blood Droplet were. Having finished my first playthrough I can tell you that there are plenty of these around (mostly sold by merchants but some are also found as treasure).

To the best of my knowledge that is it.
don't care about it. Clear dragon rot once you find a lot of people and know where they are and you have beat a major boss. Go back talk to everyone, buy more cures and then continue to die a lot and as much as needed. Find more people, kill a major boss, clear curse and talk to everyone. Its really not that punishing.
