Sorry to hear that is happening. Just to be clear, this is happening only in Sekiro and for both headphones and speakers? If so, when you have the game open alt+tab and check Volume Mixer again.
See if it is showing any audio coming into Sekiro and make sure it hasn't muted the game. When you plugged in the PS4 controller was that while the game was open or was it closed?
Lastly, I know this is simple and you've most likely already done it, but have you tried restarting your PC?

I alt+tab out of the game and the volume remains unchanged, I also tried in windowed mode. Not sure what exactly you mean by audio coming into Sekiro but when I look into volume mixer while the game is running it appears that there is no audio coming out/in.
I plugged the ps4 controller in while Sekiro was running. Another thing I tried was putting headphones into the ps4 controller which did not work for Sekiro but I worked for other applications.
I have not tested every game in my steam library but this issue seems to only be happening in Sekiro
I do not know for certain if plugging in the ps4 controller is the source of my issue but its the only thing I have changed since starting the game.

It seems to me that the issue is the game not outputting any audio is there any way to reset all settings to default? Through a config file perhaps?
Maybe when reinstalling the game the reinstall is not 'clean' so to speak perhaps it reinstalls whatever is causing the issue?
