The first stage with Isshin I am able to get him to do his thrust attack that allows me to do a counter.
He doesn't have any unblockable regular attacks far as I know so dunno how he's getting through your guard unless you stop blocking just before he hits, happened to me sometimes with failed deflects.
As said above the umbrella is great for denying his AoE slash and the chip damage that comes with it.
Also try to only heal after a mikiri. One of my early pitfalls with this fight was getting destroyed whenever I tried to heal normally. You can also make use of pellets instead of the gourd and then block until your health is back up.
For his 4th phase, watch out for his lightning attacks, that's what you're truly after, deflect those by pressing jump and attack and it'll stun him and you can do more damage and does ♥♥♥♥ ton of posture damage. don't over commit tho hit him like 4 or 5 times and deflect right away.
The only attack i would say is the hardest to deflect is the one when he starts with two sword slashes followed by a flurry of spear slashes, ending in a mirikiri counterable attack. The fast spear slashes is always what gets me.
4th phase is pretty trivial with lightning deflect.
