Using items straight out of your inventory

I remember being able to use items straight out of my inventory like remove poisonings and what have you and not have to put it on my action bar.
I like it this way because I find the action bar, clunky and hard to use when you're trying to dodge and move around and avoiding attacks for some reason it no longer works..
because it no longer works I almost never use any items anymore except for my heals and that's it..

I just heal through poisons and try not to get poisoned in the first place because I find myself getting killed trying to navigate my items while dodging and blocking and what have you

What I'm asking is how come I'm not no longer able to use items out of my inventory menu no more and I'm doing something wrong or what

you can do that. go to your inventory tab, hover the item and press use

Then go to your inventory screen and stop trying to select them as Quick Items

Sounds like you keep selecting the quick inventory slots whether something's assigned to it or not.
Navigate one inventory tab to the right and you're at the desired list of items.
