These are pretty obscure use cases and ultimately not really a better option than just..disengaging.
And yeah, its usage is definitely obscure. It's not something you would likely want in your quick item slots.
Not sure if I am understanding you. You can get another resurrection by using the hidden tooth?
The ability to kill yourself is suspect at best. While enemies tend to put away their sword and walk off, if you don't resurrect yourself you'll eventually just die anyway - it costs a resurrection token, but doesn't apply a timeout period. IT IS possible to die and wait long enough for enemies to move away from you in order to sneak away or get another stealth kill, but it's not reliable at all - sometimes enemies will just instantly aggro again the minute you stand up.
Why they didn't add a ninja rebirth item to the game, legit ninja log, that you could probably use alongside it so when you died you exploded into a cloud of smoke, hiding yourself from enemy sight like the bloodsmoke ninjutsu does, allowing you to escape, I don't know.
Very uninspired item indeed.
I consider it valuable only for the case of accidentally getting into an area you shouldn't, and being stuck. I.E if you somehow escape the tutorial area at the start of the game, and head to Castle Ashina, you can trigger the boss fight with Genichiro despite not having even rescued Kuro or even had your arm cut off yet - but Genichiro won't be there, you'll be stuck in an eternal boss battle, and you can't even kill yourself etc, which makes the Bite Down death pill a very useful tool indeed for cases like that.
Also note, if you get stuck in a falling animation either by falling through the world or getting stuck on an object or terrain glitch, you'll float for about 30 seconds and then just die (with a unique death scream too) as well, with no chance of resurrection but 100% chance of Unseen Aid. It's a neat little thing the developers included just in case.
I'm pretty sure a miniboss is simply programmed to detect you though which will then alert everyone else.
However if you use Bite Down/Tooth, you don't have to have killed anything to have access to the additional resurrection.
