Is it canon that its only take Wolf 1 try to beat Genichiro ?
I don't think Genichiro ask about his grand father's health and being surprised by the fact that Wolf can die twice 50 times each . Also Wolf can still revive him self and retry after being killed by Genichiro mortal blade that can harm Kuro .

I never thought about this, but good that you point it out!
I'm trying to think about a lore reason why this would be the case, but I can't come up with one, that's really strange. I hope someone else has an idea why Sekiro doesn't die permanently by the black mortal blade of Genichiro-sama.

I think the black mortal is a little bit different than the red one. In the lore note "Black Scroll", the black mortal is named "Open Gate", allowing a wielder to open a gateway to the underworld, "creating" life by summoning dead souls like Geni did in the last boss fight. The Seems to be its main function instead of killing the undying, this maybe the reason why its doesnt have properties the red mortal, the Owl can even hold the blade as a mere mortal while sekiro died instantly when he first drawed the red mortal.

actual death (back to idol, not just ressurection) is pretty much entirely ignored in terms of cannon, so it can be said that any death is non-cannon.
Probably an oversight. Let's just assume we never died fighting Genichiro

Probably the only "deaths" that really happened lore wise are getting stabbed in the back by Owl at Hirata Estate after defeating Lady Butterfly and getting your arm cut of by Genichiro-sama in the beginning of the game

Was that the flashback?

Yes, it's the memory/flashback you can get to via the Young Lord's Bell Charm.
How come I had the prosthetic?

There isn't a confirmed explanation why he has the prosthetic in the memories and what exactly they are. So nobody knows for sure, but I would explain it this way.

It's not just him remembering his own memories, it's more like experiencing the memories of another person through Sekiros own mind. I imagine that Sekiro thinks about what happened there, guided by the bell charm, and replays it in his mind with his current appearance and equipment.
He can also imagine an alternative course of the events that happened there. That's also how Sekiro is able to fight his own father, the Owl in Hirata estate. In the real world, he never fought him there, because in reality his father just stabbed him in the back.

Of course, gameplay is another big, maybe even the most important reason why he has his prosthetic in the memories.
