I did. I don't fully understand what he means. Do I need to go through the sewers? Are there mini bosses I need to kill first?

Did you meet the Tengu in the tower near where you fought the guy on the horse? You have to complete his “rat” quest before Isshin will show up in the castle watchtower.

And you have to talk to Isshin before you can really progress.

I am realizing this is not the type of game you put down for months and come back to. When I came back I had to work really hard to get back into the rhythm and I just forgot where everything is.

At this point of the game, there are 3 key items you have to collect. Like getting the Lord Souls in Dark Souls 1. These are to gain access to the Divine realm so you can allow Kuro to no longer be immortal.

These items are:
The Mortal Blade at Senpou Temple.
A white flower in the Sunken Valley.
The shelter stone in the Ashina Depths.

They also mention a branch from a magic sakura tree, but you don’t have to worry about getting that one just yet.

You can go after them in any order, though I’d recommend going for the sword first, as it’s the easiest.

You have to go through the Abandoned Dungeon to get there.

Really at this point, the game opens up and you can just explore anywhere.

Hell you could explore almost anywhere even before that, Kuro only gives you one key if I remember correctly.

Kuro doesn't give any key, it is a pickup in the library and it opens the door after the gun fort idol.

If you didn't talk with kuro he won't allow you to open the window there to "find" isshin in order to progress.
