If you already own Outward but no DLC, then you only need to buy The Three Brothers to own Definitive Edition. The Soroboreans is included in DE by default, so only purchase that DLC if you wish to play it in classic Outward.
EDIT: Refer to this discussion for more useful information.

So I received my copy on a Humble Bundle way long time ago but didn't play it much, this was before the DLCs were available. When I look on the Steam Store it shows that I own the game, just not the DLCs. Also the game doesn't show up in my Library, however I can launch it from the store page. It doesn't say anywhere inside the game that I am playing the Definitive Edition, so I am confused.
Do I still need to buy the 3 Brothers in order to upgrade to the DE version or do I already have it? So confusing... THX
If you don't own The Three Brothers, then you do not have DE, ignore what the store page/library show.
In order to upgrade to DE all you need to do is buy TTB DLC, Soroboreans is included in DE.
Graphics-wise, not too much. I think they did do some minor tweaks here and there, added a few new face options to character creation, etc.
But the major changes came with the DLC content being spread across everywhere. So you'll find stuff that was introduced with TTB in the base region as well as Soroboreans, you'll find Soroboreans stuff in the base region and TTB areas, etc. They also made a few tweaks to some of the items to change what bonuses they offer.
This does include corruption. It's not a major issue, but you'll need to really think about how expensive it is to get those corruption-reducing potions before spending a lot of time in corrupted places.
They also added a new unique enemy to each dungeon area (Blister Burrow, Ghost Pass, Dark Ziggurat, etc) that is gone after you kill it the first time. I do believe they're guaranteed to drop a specific type of item, been a long while since I've last played.
