The stuff of legends

Hey,
im having a bit trouble understanding what the achievement "The stuff of legends" is or how to do it.
I assume an "artifact" is just a unique weapon, but how do I "awaken its ascended form"?

this can only be done with some items in caldera region. they arent found anywhere else in the game.
