Do you have to start a new game to play DLC?
Or you can play it with an pre-dlc character?

No, you do not have to start a new game to play the dlc!
You can travel to the dlc area even with old characters as long as you own the Dlc and have enough rations and money.
To reach said dlc area, you need to find a caravan (there is one in every major city, as well as randomly spawned in the world) and ask to travel with them.
