You'll generally find small sapphires when mining iron ore veins, particularly rich iron ore veins.

I have found like 30 or so already, this sapphire was for to gain entrance to a loot room that has 2 doors both need 1 sapphire each, the dungeon is the one you are send to get the shield mushroom at the start of the game.

It's random where you find them. You can find them on dead baddies or from mining, but all loot that is not found on the ground or from a unique boss is completely random.
