For the capitalist head, it will be easier to understand this: there are not 2, but three currencies in the game. RUB and USD for foreign trade by goverment (player). The third currency is non-convertible, used only in your republic.
"Realistic" mode is "realistic" by comparison with the other modes. If you want a "realistic" game by comparison with reality, well, that already exists. It's trying to kill you, and it will eventually succeed. Put that on the steam store page, see how many takers you get.
I do wonder why they didin't just use the fan choice of name for it, cosmonaut mode. I have this uneasy feeling that some lawyer told them using any name they heard first somewhere else would expose them to unending legal grief.
So don't take it too literally.
And what is considered realistic now is subjective after all. Example: North Koreans...
Ok, but this is not comunism, this is kibbutz. In realistic mode you cannot use money as a "resource" to build stuff. You need to go buy resources and build the thing you want. Cool i say, exactly how it is in real life, but workforce is costing.
In comunism, in which I lived for that matter for a short while, and my parents for a much longer time, if the great leader, said, you will work for the country to build this, you had to work for the country, with or without pay, depending on the job and your "importance" to the project, but, you still had to pay for your own bus ticket if you went from the power plant build to the shopping center to get your food, in some cases free or electronics which were not free at all, most of them ( not even radios were free to listen to the "contries" best comunism radio ).
Now, don't get me wrong, this game is challenging and i had tried a few times to get a successfull start and understand the mechanics of it, and applaud the devs for making this "realism" mode that presents a challange and is almost there. But, it's not perfect and they can tweak it to be a real life simulation.
These suggestions, are just ideeas and i saw ppl have some good stuff, like dissapearing ppl from busses from border to construction place, damns that produce electricity, petrol consumption simulation, to just name a few.
Realistic does not even attempt to make things like citizen needs realistic: Water usage, fueling vehicles, etc. are all separate settings.
There are two ways to interpret free transport: One is that in this game's republic it's really free, or paid for by some non-monetary unit like vouchers. Another is that the RUB balance is only your external trade balance, while the internal treasury is hidden from the player.
You also don't receive rent or proceeds from shop sales. Meanwhile you don't have to pay for ongoing maintenance of vehicles (like spare parts) or pay salaries. So I like the hidden internal treasury explanation more.
The starter towns are clearly a game balance thing. The game doesn't generate proper services in those towns, so if they acted like normal citizens they'd all immediately leave, which wouldn't be very fun. So they get a temporary freeze on happiness until you start building there. You can rationalize it as people having higher expectations once their town starts to be developed, but really it's there to prevent strange behavior in pregenerated towns.
Winter starting on different months is a good idea I think. It should be very easy to do and would make things more interesting by making winters less predictable.
The truck drivers are a known problem. You can also complain that they drive around forever without needing to take a break. I guess the fix would be to make drivers walk to the vehicle depot, take the truck, drive it around and return it to the depot at the end of their shift. Then if you cannot provide workers to your depots, your vehicles don't run. It would also add a better role for the depots. I really wish they implemented something like this, it would make realistic mode more fun. You'd have to allow foreign workers to walk from the customs house though, so that you can get things started in the beginning.

Realism: fact of representing a situation accurately or in a way that is true to life.
