The pollution model is pretty simple in this game. All pollution becomes "air pollution" that manifests as a cloud floating around the source, with the center having the most concentration.
If I had to guess your issue, you either have overflowing sewage in your city, which creates pollution when dumped, or your trash is overflowing at some residences (if you have waste enabled). Hospitals also produce hazardous waste, which creates a lot of pollution when dumped.
I have seen the pollution 'cloud' from this extend upto 800m from the discharge, but that was when I also had chemical plants, another high value sewage pollutant 78%, running to the same discharge.
I had to build a sewage treatment plant to counter it, although I could have moved the discharge further down the river, I quite like the look of the sewage treatment plant, but it is quite expensive to run due to the chemicals requirement.
It's worth the cost for the eye candy I think :)
