Loyalty gets raised to a maximum which depends on difficulty and monument quality. For instance on medium difficulty the worst monument (2.2%) increases loyalty up to 40%. The best (4.5%) will raise it further up to 43%. So I assume its linear:
max. loyalty from monuments (32/42/52) = difficulty (28/38/48) + monument quality (2.2-4.5)
I am pretty sure effects of different monuments add up. I don't think adding more of the same adds up, and if it does, it has rapidly diminishing results.
What do you mean with other buildings? Only those in range get the benefit.
Mouse over the info window of a monument and all buildings in range get highlighted and I observed a loyalty gain in all buildings. After almost a year since loyalty update I even spotted it once on a bus stop.
Regarding a loyalty drop, there are many things that can reduce loyalty: no needs met, pollution, crime, unemployment, no power/water/heat ... An invited worker will come with a random loyalty and will adapt to the living standards over time. Often this means they loose loyalty. Monuments are the first things I build to counter that (up to a certain maximum), radio, tv and uni thresholds come later after I solved all the causes for a drop.

Workplaces do light up, the range in the tool-tip is from center of the (workplace) building to the center of the monument. Many factories are just really large and a range of 65m may not cover the building. Just to make sure: I am not assuming it works there, I actually observed a loyalty gain in all kinds of buildings.
Loyalty gain in general is a slow process and having monuments at residential area and at workplace will help (up to a certain maximum). But people and their kids spend a lot of time at home, so I place the first monument there and eventually build one at workplaces too.
