Try meathooking straight at the blood maykr and firing a PB or the ballista without adjusting your crosshair, should guarantee a headshot. You could also microwave beam them when they are charging up to interrupt their attack and falter them long enough that you should be able to get a headshot. I agree that the blood maykr is one of the most annoying enemies, just sharing some methods that can make them a bit less annoying
I think they're up there for me too.
Like every demon there are several good ways to kill them quickly and efficiently, but god help you if you let them live too long or forget about one.
They are annoying decent at dodging arbalest, can dash a surprising distance, tanky enough that you gotta focus on them for at least a couple seconds to take them out, can block shots with lost souls and you can waste lock on burst on them too. they have a powerful ranged attack (80 DMG on nightmare) with very fast-flying, guided projectiles and will melt your health quickly if you let em. They leave proximity lost souls around that explode for 100 DMG on nightmare, and they actually have a very devastating, pretty fast melee attack (130 DMG per hit) that also knocks you back. You normally won't get hit by the melee but they can dash to you in midair and juggle you for 2-3 hits sometimes!
They also seem like they deliberately try to get directly above you if you're on the ground, where you can't see them without looking directly up and thus ignoring your immediate surroundings. Them being right above your head also blocks you from getting up into the air.
I think they are a well designed enemy, though. they are supposed to be a PAIN
It's all about what is the most annoying or what would you change if you could!

Every enemy in DE that has a mechanic has a very very stiff one.
That's why my pick here would be "Turret".
Nothing screems more Doom Eternal game design mentality more than this enemy.
"Waiting for the enemy to be vulnerable and then shoot it."
When every enemy is a sponge, guns feel weak. When a big tough enemy is a bullet sponge, that is expected. When a once-per-level big and tough blue glowing Baron of Hell is a bullet sponge, it’s okay because he is the boss of this gym.
let the demon be able to 1 shot me if thats the trade thats needed. i'd much rather have that than increased EHP
As for monsters one-shotting you, I’ve almost finished Turbo Overkill recently and it’s a very fun game, but I dunno how to feel about the abundance of OHKs on max difficulty. I understand it’s not supposed to be fair so I don’t complain, and the loading is very fast, but I think I prefer the pace of Eternal where some enemies can take a beating but so can Slayer.

Turbo Overkill is a DE-inspired retro FPS with a cyberpunk theme. It has pretty fun movement and a chainsaw leg slide attack.
