Taras Nabad - Several extremely difficult fights, very long level and requires you to eliminate high threat demons quickly.
Urdak - Aside from Final Sin, 3 of the hardest fights in the game. Will really test your ability to weapon swap, prioritize targets and stay mobile, or strategic use of heavy weapons.
Final Sin - The most difficult fights in the game.
Those are my top 4 most difficult levels. If I had to pick one it would probably be Taras Nabad with Cultist Base in second.
The only slayer gate you need to complete to have an easier run is the first one, Plasma Rifle with heat blast is really useful since there are a lot of Imps, soldiers and gargoyles. For the revenants you can use the Bolt Action mod to take out it’s weak points and as for the Dread Knights, keep your distance and pressure him with the Sticky Bombs or Micro missiles.
The only mod I mastered without using a token was the Meat Hook, that thing is indisputably the most useful mod specially for a Nightmare/Ultra-Nightmare run, not to mention it is easy to master as you can do that before The Super Gore Nest. Then, I used tokens for the Micro Missiles to deal with the Icon of Sin, the lock and load for the Khan Makyr and the arenas in Final Sin and Urdak, and the airblast to deal with Pain Elementals, Revenants or Cacodemons quickly.
The First Phase of the Gladiator is not hard, if you are able to stun him, get behind him with the Rocket Launcher and target him with the Lock and Load, when his stun animation ends fire the Lock to deal a lot of damage. The second phase is where things get messy, he attacks really fast and if you get cornered by him you are going to suffer a lot of damage unless you are able to dash. The weapons I used the most were the Ballista, The Rocket Launcher, The SSG and the Chaingun. If he spawns demons close to him throw an Ice Bomb and then throw a regular grenade to finish them off, works better if you have the Deal More Damage to Frozen Demons perk which I recommend to get as soon as possible because to survive in UN you need to use the Ice Bomb a lot.
As for the Icon Of Sin.. pay attention to what attack he will make because one punch and you can die, only use the BFG when he is standing still. The most useful weapons to fight him for me are The Heavy Cannon with the Micro Missiles Mastered, The Chaingun, The ballista and the Rocket Launcher. His second phase is not hard, right after you enter the portal fire a BFG at him and get out of there before he smashes you. If he throws meteors at you or his “death ray “you can easily go back to cover.

Super Gore Nest through Mars Core isn't bad because you have a lot more gear to work with. Things start to spike a bit in difficulty once you hit Taras Nabad, but it's manageable as long as you know the combat loop well, and you have important spawns memorized (like the archvile)
All of the boss fights can be pretty challenging, but they can be cheesed to a degree
Your frag grenade and heat blast mod can falter the doom hunter. If you have the double grenade upgrade, you can nearly stun lock the doom hunter during his first phase if you time your grenades well. Once his shield is down, the lock on burst on your rocket launcher quickly takes both of his phases down. Just make sure you're chainsawing frequently
Use either your super shotgun or ballista on the gladiator when his eyes flash green. Once he starts flashing blue, go behind him (or to the right side of him if his back is covered) and launch two lock on bursts at him as soon as he stops flashing blue. Since the shield mainly protects his front/left side, the rockets should hit, and they do massive damage. You can repeat this process to take him out during his first phase with minimal effort. The same tactic works during his second phase, except you can attack him from the front since he has no shield. He's also vulnerable while he's using the jump rope attack, so take advantage of that.
You can take out the khan makyr's entire health bar with two lock on bursts and a ballista shot. Just try to be quick, since her health regenerates, and try to stay in midair as much as you can to avoid hazard damage. When you need to refill your ammo, just use an ice bomb on a makyr drone or better yet, quickly swap to the chrono strike rune and precision bolt mod.
The BFG works incredibly against the icon of sin, and a direct hit can single handedly take out an entire body part. During the first phase, an animation plays each time a body part is destroyed, and he's unable to attack you during this animation, so the best strategy is to target specific body parts, and deal as much damage as you possibly can, leaving him no opportunity to attack you. Similarly to the khan makyr, staying in midair is a good way to avoid hazard damage, and summoned demons (unless he summons cacodemons and pain elementals). You can also use the chaingun shield mod to tank his fist attacks. The body part animation doesn't play during his second phase, but the strategy doesn't change much. Just deal as much damage as you can, and use your BFG shots carefully because you want to land those directs. Watch out for the attack where he shoots a pentagram out of his head, because it'll home in on you and it can be tanked with the chaingun shield. And while he's using the meteor attack, go to the bottom middle section of the map and take cover.
The master levels are pretty challenging. They're a good way to test your skill and/or improve at the combat loop.
Urdak can never make it past Urdak on ultra nightmare ...maybe one day...
TBH the gladiator is really easy just replay the mission many times and you will figure it all out .... i be more worried about the icon of sin... just so much ♥♥♥♥ going on at once its not hard to get hit or killed by heavy demons around you as your trying to destroy the body parts...
