Bluetooth headset bad quality fix
Go to System/Sound and check/uncheck the "mono sound". Instafix. No system or game restart (Win11).
It worked for me. I hope it works for you

This method does not completely solve the problem. Each time you start the game, you need to minimize it and do these steps again, which is quite tedious.

Press "Win+R", enter the command "devmgmt.msc" and click "OK".
In the window that opens, select "Sound, video and game controllers", find your headphones with the signature "Hands-Free", right-click on it and select "Disable device". Restart your computer.

Unfortunately, because of this, you will no longer be able to see the battery level of your headphones in Windows.

Please note that you only need to turn off the hands-free device, you do not need to turn off your main headphone device. Also, do not confuse that it is necessary to disable it in the "Sound, video and game controllers" item, disabling the device in the "Audio inputs and outputs" item will not lead to a result.

A little theory about what we did:
Hands-Free or HFP is a Bluetooth profile that allows you to communicate through the microphone built into your headphones. Unfortunately, this profile only supports mono audio, at a low bitrate, so it's not designed for simultaneous gaming and chatting - the quality is too poor, so gamers usually use either a wired headset or a separate desktop microphone.For some reason, in Windows 11, this profile is automatically enabled in some games that check for a microphone, even if the hands-free device is not set as the primary device or is disabled in mmsys.cpl. This behavior was not in Windows 10, so let's hope that Microsoft someday fix this problem, but for now, this is the only way to solve it.

I did this but now there's no sound coming at all from the game. The headphones work normally during the logos/intro and by other games. The Game only emits sound when i have HFP enabled

This is really, really poor for a triple-A title. I tried everything, including disabling all 'headset' devices, setting the default device to headphones rather than headset and every time I start Doom Eternal it switches to mono-audio. How did this even pass a superficial QA?
The fix for me turned out to be turn the mono audio switch on then off again. What a stuff up - almost refunded the game because of this.

Because its a windows problem. If you turn on an app that requests use of a microphone and you connect bluetooth to it, windows forces the BT device into call mode. The game does not have permission to do this, or even stop it. Windows is the problem here. This will happen with literally every application. From any multiplayer game, to discord, to your internet browser. The moment anything you run asks windows if there is a microphone connected to your PC windows throws all BT devices into call mode.

A windows problem can only be fixed through windows. That should be common sense.
