Radeon HD 7950
The GPU doesn't produce these symptoms in other (also graphics-heavy) games.
Could definitely have something to do with how well the 7950 supports Vulkan...
Although it looks like a driver issue, since I found someone else's post about the same symptoms:

The poster above has a R9 280x, and I have an HD7950 - both have been labeled as "legacy" by AMD back in September.
So this issue is unlikely to be fixed :(
Downgrading the graphics driver to 21.2.2 solved the issue.
Also, I tried Nimez's modded drivers for legacy GPUs, works like a charm:
