Game completion on Nightmare

What do I have to do to trigger the nightmare completion achievement? Do I have to complete the game from start to finish on Nightmare without changing the difficulty or can I just raise the difficulty on individual levels after I've already beaten it on a lower difficulty?

Thanks for the answer! Unfortunately I wussed out and lowered the difficulty at Taras Nabad so it looks like a new play-through is in order.

You actually can. Otherwise I don't know how much longer I would have been able to persevere on that nightmare of a difficulty.

There's a nightmare completion achievement? I've beaten the game on Nightmare a couple times and all I received was a sense of pride and accomplishment.

I think there is only a Ultra Nightmare Achievement. Apart from completing on any difficulty achievement.

There is no achievement for Nightmare or Ultra-Nightmare. I don't even think there's a milestone for Nightmare completion, only for DLC 1 there is milestones for that.
