As far as I know, they still represent demons that killed other players - but *only* who have killed other players on your Steam or Bethesda friendslists.
If you don't have a lot of active Eternal players on your friendslist, then yes, of course you'll see the same empowereds again. They haven't been playing, and hence haven't been dying, in order to change what you're seeing.
I have a large friendslist and a lot of active players - the empowered demons are almost always different except for a few particularly tricky ones that everyone seems to have trouble with.
Possible, but still unlikely. We know the system worked as intended previously, and if you have the console unlocked, they still get player names assigned so I'm going to assume that it still works as originally intended.
Might generate them randomly if it has no pool to draw on, but even now - over the last month when there was a general lull in playing, I saw almost no empowereds when running Master Levels. Now that a hundredish friends are playing again, the maps are full of them.
They are somehow tied together still.
Its like the devs were only looking at speed runners and all the tricks those guys use and tried to cater to that, instead of the majority of normal people who play the game...
You can disable empowered demons...
I can say with 100% certainty this is not correct. I consistently get empowered demons as early as Super Gore Nest.

Cynic gets the exact same Caco I described in my original post. Seems a bit suspicious, if you ask me.
Again, I have nothing against empowered demons, I think they're cool. But I'm a bit annoyed ID seemingly couldn't implement them as they intended and then just kept consumers in the dark about it.
