In the Cultist Base mission, right after the scythes when you open the door with the Cultist key the player can't progress because the game still thinks demons are around when there aren't. (Loading the last checkpoint, starting a new campaign, looking around in the level and reinstalling the game seems to have not worked thus far.)

Wait for it to be fixed or start a new campaign.
When using Fast Travel on certain missions certain doors remain closed because they are scripted to be closed after going through them the first time.

In the Super Gore Nest mission, after finishing the second encounter with the middle stomper and the two monkey bars on each side the teeth door at the top will not open.

The IDKFA cheat seems to break weapon mods when used sometimes. I've had this happen myself, where the shotgun grenades stopped exploding, only staggering enemies, and most recently the micro missiles attachment for the heavy cannon are instead the scope shots, while still using the same amount of ammo as the missile shots.
It seems like when I was posting the bug you're talking about here it didn't post itself. Internet problems maybe. Anyway, thank you for posting this bug in my place but I would also like to add here that it's been reported (by the user Please, Hold!) that the Chaingun Turret mod is also experiencing issues similar to the ones you mentioned like when using the second fire instead of the turret being used it pops up the shield from the Chaingun's other weapon mod.
When in-game, after playing for a few minutes the audio completely cuts out.

2: video: the chromatic aberration always enables itself, or a disabled state cannot be saved.
3: Lost Souls can be stuck even inside objects
4: I have seen some videos, where the player had only the slayer's hand on the screen, but the weapon were missing.
5: Many key bindings cannot be changed: Cutscene skip, photo mode, Revenant Drone in single player
6: If it is not planned to add another equipment, then it is stupid to have a use equipment and a switch equipment button. It would be better to have a grenade and an ice bomb button.
7: if you alt-tab out from the game, and alt-tab back in, you must press alt once otherwise the tab does not work, and you can't open the dossier with it.
This isn't a bug though.
4K resolution results in colors constantly shifting
I made a separate thread for this topic before I found this thread

When in Battlemode weapon order when using "next" or "previous" keybinds or mouse scrollwheel are not in the correct order, e.g. hitting next weapon from super shotgun gets you heavy cannon instead of ballista as in the campaign.
I would guess most people use specific keybinds for preferred weapons, but for those who use wrote memorization of button taps or wheel clicks to dial in weapons, the change in order is a massive pain in the ass.
I too found door ways on both sides of a particular room in the Fortress of Doom was acting like an invisible wall for a specific period of time too. It did not behave like that from the start of the campain, and it was resolved before the end of the campaign. So there seems to be some section in between that this happens for a specific duration, like after completing mission A but prior to completing mission B.
Thanks for mentioning this, I'm not the only one who was wondering why the Doom Slayer cannot properly walk around his own home basically lol
