Am I missing something here?

"Activate all Combo Visions"

There are 10 CV's total - 4 for Yuito's platoon, 4 for Kasane's platoon, and then Yuito's and Kasane's Combo Visions.

Used all of them on the same savefile - no achievement. What gives?

Yeah, far as I know you just have to activate them, doens't even have to hit an enemy, but I finished it on an ex game from my first play-through. First did them all with Yuito as main then switched over to Kasane and did them all again. It should work. You do have to activate them all between the two. so essentially twice around.

Not for me. I have two separate saves for each character and I did all the CV on Yuito then load up Kasane for the last one and I got it.

activate 8-9 vision at 1 tome, one after athers
