Once all my lives are gone the game keeps going. How do I restart the level over or go back to the main menu? I currently have to force close the game and reopen completely. It mentions saving checkpoints - how do those work?

I completed the levels, however the achievements did not unlock. Are these broken? Why is there no Options menu? Cant kill the music, can't set up the keyboard or controller properly. Also the default keyboard layout is about as unergonmic as you can get. WASD plus Only the Left CRTL to fire, Space bar would have been a bit better as you could at least use two hands. Glad this was only $0.74...
