You could try applying custom Windows CPU affinity settings to the game, forcing it to use only max 4 cores or so instead of all of them. This should help if it's a multi-threading issue.
1. Is the pause-forward loop repeating regularly, at something like one second intervals? Or does it depend on the speed of the train or something else?
2. Does doubling the simulation pace (press page up) change the behaviour?
3. Do you get rubberbanding when turning the camera and moving about in the cab, or is it just the train motion?
4. What does the "sim step ms" value say in the F1 info box?
Yes the rubber banding speeds up with the game speed.
No the interior view movement is as smooth as silk.

Also in the test run with the dmu the Sim seed was only around 1100+
