The only items you will never lose upon death is the Default Items (The starting flashlight, dots, etc. that is always loaded into the truck loadout) - It should also be noted that the Default Items respect your tier unlocks (so if you equip the T2 flashlight you will have one by default)
Speaking of tier unlocks - the items cost the same regardless of the equipped tier level, and all items you have in storage will all be on that same equipped tier level. So there is no extra risk involved for taking in higher tier items over lower tier ones.
A death can be as costly as a couple grand... but that's at the extreme maximum; on easier difficulties you get a small refund in the form of item insurance.
If you get out alive but don’t manage to identify the ghost then you don’t lose anything except any “consumables” you used. If it’s all going to hell, get out rather than sticking it out and becoming dead.
