I had a similar problem, and so did my sister, And that was the fix, dumb as it sounds.
Is windows speech recognition running? Is Phasmaphobia the active window or did you tab into Discord? Have you checked the sound settings to see if windows is trying to utilize another microphone?
See if your friends can hear your proximity chat instead of discord's voice channels.
This is a known issue since forever and has yet to be fixed.
It's important to remember that everyone has a different setup and accounting for everything that can go wrong isn't easy. Plenty more people not having issues after all.
If you've already tried my other suggestions, well I just found this in Windows
What's the input? Specify it to your microphone. Especially ensure this is the case for Windows Speech Recognition (labeled Speech Recognition in this instance)
Beyond that solution, I diagnose your computer with aids and sentence it to a formatting.
Have you or anyone else done anything weird to Windows since first running it, like trying to strip out it's telemetry etc? Maybe something weird is happening in your privacy settings.

Seems like some weird funky things like running Steam in admin. mode bugs it haha.
