The bot_defer_to_human_items command only matters when you're playing on the Terrorist team on a bomb map. If it's set to 0, then a round can start with a bot having the bomb. If it's set to 1, then a human player will always have the bomb at the start of every round.
Also, if you go into csgo\cfg folder, you will see some files that start with gamemode_ and end with cfg. Those are the files that contain the settings for each gamemode. You may want to look at the casual, competitive, and demolition files. You can open them with Notepad and maximize the screen. Near the top, you will see the bot_defer_to_human_goals and the bot_defer_to_human_items settings. You can change them to whatever you want for each gamemode in each file.
Another thing is that sometimes when we get a CS GO update, those gamemode_(whatever).cfg files are reset back to their defaults. So that means that we have to go back into them and change them back to our liking
