They were actually added in game but due to amount of experience needed no one got them. Later, Valve refused adding black medals as even red ones were nearly impossible to achieve by legal ways only. So you can only see its textures in game files but you'll never meet anyone with it in their inventory.
And yes, upper guy stated right - each year there wasn't more than 2-3 red medal owners, and almost all of them were later banned for cheats/XP abuse.
