Also, check if you can bind the buttons to other actions inside the game. For example, drop weapon with MOUSE 4, and try if it working; if works, then bind it to any action you want and try again.
What makes me assume this has to do with CS:GO is the fact that this phenomenon only appears ingame.
Do the MOUSE4 works for drop a weapon? or to reload? (setting it from the Game Settings menu). If it works in the game, then the bind of use weapon_flashbang has some problem.
but, iff the button stopped working with the game, I should look on the mouse software to see if it is using another profile, or something like that.
I use a Logitech M500...Logitech themselves say on their homepage that this mouse functions without any Software (therefore you can not download any drivers or software for it/ the setpoint software is only recommended for mice with 6 or more buttons). But as I already mentioned, the problem still remains if I use a different mouse...
Before binding it with the console, the game must recognize it on the game settings.
Did you already tried to delete (or rename) the config.cfg file?
SteamApps\common\Counter-Strike Global Offensive\csgo\cfg
It will reset your bindings.
If the buttons works outside the game, then ignore this one: Go to Windows Device Manager, right click and uninstall the mouse (you will need another mouse). Disconnect it, and reconnect it to another USB port.
I'm out of ideas.
