You don`t have to use cash. Cases or skins are unnecessary to play, it`s just kind of customization but with no fight bonuses or boosts. You can spend money to have beautiful weapon skin or to open a case (with a chance of beautiful weapon skin inside) but just for Your good humor, nothing more.
By the way, case could contain weapon skin worth 0,20€ or a knife skin worth 360€.
Weapon skins an cases also drop at random after matches.

cases keys and skins are all cosmetic and optional. They cost real $$. You can however sell extra skins & cases you get randomly for $$ and then buy what you want.

i heard of alot of people who spend a lot of money, to load their steam wallet to open masses of cases. remember, 1 key = 2 euro.
I only spent real money to get csgo and to load up my steam wallet for 5 euro. since then I only buy skins/cases and buy new ones with my virtual money

You don't have to spend anything. Weapon skins and cases randomly drop at the end of matches.
