3. 14 days is max but your cooldown level still go higher and higher

Also griefing cooldowns have it's own separate level counter. Griefing cooldowns start at warning message, then 1d, 2d, 3d, 4d, ... and 14d cooldown the maximum. Griefing cooldowns, unlike regular ones, don't reset over time.
You can be permanently banned only for cheating, boosting or a client automation.
None of regular or griefing cooldowns can lead to a permanent ban.
You mentioned that 14 days is the maximum (Level 4), but then said it can go higher? That doesn't make much sense, lol
they are a set 20 hour cooldown always.
2. There is no set number of times before a permaban.
You can however be handed a overwatch ban during the duration of the cooldown.
