mills and thera are extremely bad for this - they're the only maps that have instakill triggers you can easily stumble into by strafing slightly too far
in fact i think none of the other maps have kill triggers at all. they don't even have water deep enough to drown in. but on mills you can walk off the dock into an inch of water and die instantly costing your team the round .. or you can get nudged by a teammate strafing into you and have that happen. feels extremely amateurish.
thera has a lot of places that really feel like you should be able to stand on a certain surface (like the railings of the stairs going from outside T spawn to B site), but you can't because the clipping on them is so atrocious (so you try and just end up making a fall sound and then getting killed)
i get this sometimes too, and it always ruins fights that i may or may not have had a chance in. not sure.
there was talk at one point that inscriptions on guns using asian characters would cause this (not joking) - i've asked around trying to see if anyone ever proved that or if it was patched, or if the real trigger for it is something else..nobody seems to know
might explain how random ak and mp9 kills dominate so much of the game - the other person is frozen like a deer in headlights while they get killed..
but i agree both these things happen a lot in my experience too!
