Err...they use High for "Effect Detail" in the video settings; it gives better vision through molotovs and I think helps the scope blur.
You can leave your resolution alone, 16:9 is perfectly viable. Ultimately 4:3 stretched is the best resolution for raw aiming since it widens models, but screws with your sens otherwise. Resolution & etc is mostly personal preference, one setting isn't better than the other settings in every way. *edit* make sure you have "Effect Details" on high, it helps see through molotovs.
As far as crosshairs go, you generally want something small and visible. I'd recommend having it static (stationary) when you shoot and having it move when you run if you want. I feel having a crosshair that moves when you shoot encourages a conflict between muscle memory and visual feedback...but that's pretty anecdotal advice, so take it as you will.

This is a pastebin of all the cvars in my config.cfg with "crosshair" in them, if you want to try mine (can be seen in the images above). I use 1920x1080 so you may want to match my video settings or it may look different; I don't know.
