That is why in overwatch you don't see chat or hear voice comms, because in a situation like that a teamkill is a teamkill.

What evidence is available to the investigators?
Investigators are presented with a replay of a randomly selected eight-round segment from an accused player’s match, and their task is to determine whether or not that player has committed any offenses during that replay. The suspect is referred to as “The Suspect” and the other players’ names have been replaced. All text and voice chat has been omitted. The investigator is expected to make a determination solely based on the actions of the suspect.
That was my thinking (I don't know anything about the process as admitted) as if one gets a ban lashing out is just as bad and deserves the same punishment.
I think that's okay, but if he did 15 damage, then (depending on the weapon) he shot him 2-4 times in the chest, which I think is way too much rage just for one flash.
I think if he was helping the team and doing good besides that one screw-up, then he can be forgiven. If he wasn't really a big credit to the team, I would mark griefing. It's your call.
