Yes there are many factors that play into missing shots. However, even if you do everything right the character can still slightly move out of your shot causing you to miss. I am not stating that this is the only reason people miss but rather one of the reasons you miss that may not be all your fault.
I tested this on legs, shoulders, nuts, facing different directions, etc... hence the 400kills
Feel free to test for yourself. The fact is, the slight moving of your character while stationary does cause some shots to miss.

This bug was fixed last week. Check ♥♥♥♥♥♥♥ patch notes.
The mouse was never moved in this video. Feel free to test it yourself.
Also, if you're worried about you possibly ever so slightly moving the mouse.. unplug it and bind something like "enter" as your fire key. You'll notice that the characters themselves are moving, not your crosshair.
I don't think people are understanding that even while standing still the hitboxes are constantly moving.
If you watch the video, my bullet never went through the ingame model. However, the ingame model simply moved out of my crosshair while stationary.
pause the video at 0:04 and you'll see that while I never moved my mouse, the model itself is no longer in my crosshair and then at 0:05, the ingame model moves back into my crosshair.
