I didn't read this huge wall of text, but I'm gonna try to simplify:
01. Download a free hack = VAC gets you.
02. Purchase a hack = Overwatch gets you.
03. Play casual = Players kick you out.
If you download a free hack and go play the game, VAC will get you because VAC only gets free hacks that are stupidly made. If you, instead, purchase a hack and go to any casual mode to use (but Danger Zone because Danger Zone has no Overwatch) players will report you and you'll banned by Overwatch, the same fate.
Instead, if you purchase a hack, and play casual modes to avoid Overwatch, players will change team and kick you out anyways because they don't want to have their casual matches ruined by a retarded hacker... Of course, being kicked out it's a 15 min ban, but the effect is pretty much the same? You won't be able to play in that server, effectively people will keep kicking you out from servers and game modes.

Oh yeah... VAC is pretty ♥♥♥♥ , probably there are free hacks out there that VAC can't get, but that's the idea behind the whole CS:GO scenario, this is the "order of events", as a default way to say.
A player faking it perfectly could bypass Overwatch? Yes and no. Since Overwatch is made by a lot of people, this guy must fool a lot of people, so it is possible for him to bypass the Overwatch at least once, but keep using will keep sending him to Overwatch, and fooling multiple different people multiple times is kinda hard to consider, so I don't think he can avoid Overwatch.
BUT... There's a problem, people are doing less and less Overwatch because there's no incentive, we don't know if it's a free account, or an expensive one. Me, and a lot of people to be honest, feel that Overwatch means nothing because we could be banning an account made yesterday, without skin, without purpose, it's like a not-worth never-ending battle.
About casual players, I had my moments to see a guy clearly cheating, everybody in the server see he's cheating, but people didn't kick him out because "it's casual, let him have fun", it was in Arms Race and it took me 4 maps changing team and talking to people to convince them to kick the guy... I imagine that not a lot of people have this will to make things right, so, yeah, it's possible that people are hacking in casual because players don't care to kick.
Do you change team to open vote kick against the guy, or do you expect his team doing it for you? I know a lot of people who cries and complains about a cheater in the other team, but they never leave their comfort zone to go there and open the vote kick themselves.

If spin bot players would be kick and report then it would be much more easier
I think the report system must be reworked.
So you can report if the enemy has clear aim bot trigger, so people in teams of such players can get vac ban if they not report the cheater and ignore his disruptive behaviour. This would at least stop all people which see spin bot players to keep them in team.
I come up with a category which is mark as clear and open visible cheat assisted by team. This category is when people ignore very obvious aim hack cheaters.
I want the anti cheat to work a little more sensitive to 5 enemy players reports, and if obvious cheaters would have a down of being report too from their team, would be great, expecially those which aim hack on face, but are getting covered by team mates. I get some cheaters can team up and not be kick, but i don't see why a fair player wouldn't report such players.
Now idk if is wrong, but i use grief report, to suggest that the player had uncompetitive misbehaviour ahead with cheating marks for wh/aim hack/ bunny hop movement. I don't know if in teory i do something wrong, i always report players around cheaters which refuse to stick and report the cheating players, this is most oftenly going under wh/other hacking, because in my opinnion is other hacking category to benefit some cheat, and wh is also possible as one who has wh can control where the team should surgery entry and take sites or protect sites.
Maybe it makes sense or not, i want a more intuitive system of report, where players which have long hours played and many overwatch cases, can counter much lower the players who cheat trust factor, expecially those which also report their team mates for cheating, should get an increase trust factor too. Or maybe consider players only which are not allowing cheats to happen in both sides. It would be great.
Also grief report doesn't come with description of what you report, so there is not accurate easy to ban such players, because overwatch players have no inside of what a team mate gets reported, from abandoning support on some positions, to refusing to defuse bomb, to going random with bomb, how can just a check point on grief explain that to anyone which is investigate a such player??
Grief button, could also work for describing and pointing cheaters, and special moments the enemy you have seen it cheating very clear. Filter points for grief as description could also go informative for players which take the overwatch case.

Not for me. I played a lot of Demolition before the update that broke the game mode, and now I'm playing a lot of Arms Race. In Demolition players don't accept to easily because they rather win carried by a cheater than lose an honest battle, but in Arms Race people accept vote kick pretty easily since it doesn't matter, it's a team mode, but team doesn't really matter.
Do you change team to open vote kick against the guy, or do you expect his team doing it for you? I know a lot of people who cries and complains about a cheater in the other team, but they never leave their comfort zone to go there and open the vote kick themselves.
