get normal netsettings and check if something in the background is taking too much ressources. if it still keeps happening then its the server.

So I was able to fix this, though unfortunately I'm betting that the fix i used almost certainly won't work for you.
I had just got a new computer and the game would have these issues, but when I turned down graphics settings, it got fixed. So it seemingly wasn't actually caused by network issues, but rather the settings being too demanding on my computer.

"The "sv" tag shows the fps of the server as of the latest networking update delivered to the client. The "var" shows the standard deviation of the server's frametime (where server fps = 1.0 / frametime) over the last 50 frames recorded by the server. If the server's framerate is below 20 fps, then this line will draw in yellow. If the server's framerate is below 10 fps, then this line will draw in red."

Mostly not if you have high packet lost and things that's you side but var and sv it's still the server side problem

there are no packet loss or so whatever. maybe sometime it goes to 20-40%. but i understand that cause when that happens i have a ♥♥♥♥♥♥ net. but sv-var is killing me. even today when i played earlier. i have stable net. my VAR goes up to 20.
