If you attack innocent civilians who have done nothing to you, they will leave. You should be careful about who shows up in your line of fire. Also you should have received a warning if an innocent is in your line of fire.
So I'd say that it's definitely a thing, but it doesn't seem common enough to deter from starting a playthrough.
As far as I can tell, this bug happens because of splash damage or friendly fire. So, if you are using something like a shotgun and some splash damage gets on a civilian - Kwon and Lucia may treat it as you attacking civilians on purpose. Or if some of your allies (not rangers controlled by you but allies controlled by AI, like in the slavers fight at HQ, for example) deal some friendly fire damage to some other allies (or even friendly turrets) - Kwon and Lucia will again treat it as "your fault" and "you attacking innocents". I think you can avoid the bug by paying close attention during battles. If you'll see some complaints appearing above your companions ("What are you doing?", "Stop it!" and so on), just reload and try again.
It was a bug and I already moved on from the game because of it. I did hurt any civilians at all, not with splash damage or anything. I would play through an entire encounter then they would randomly leave saying that I was hurting innocent people. After doing more research I found it listed as a known bug on the developers website and not fixed yet (atleast at the time). I think it had something to do with teamates taking damage and it being counted against you for no real reason. But hey Pillars of Eternity was pretty cool. Didn't encounter a game breaking bug the first 5 hours too (in fact I didn't encounter one at all. amazing when games work right?)
