How/Where do you get rad shield +3?

All 3 rad upgrades are available at your ranger HQ mechanic vendor in the garage, just do quests and they contact you that there are new uprades available. It's a bit hidden because in W2 the rad suit upgrades were most times mainquests to progress. This time its also necessary but you don't get a single word from anyone about it.

I am pretty sure i got it before that during my last playthrough.

There is also a utility item that you can put on the kodiak that protects you ftom radiation, which I think was at the dept of energy. You need an upgraded AI to use it though, so depending on your choices that may not be an option.

YOU have to have Ronald reagans "talking Car" hidden car AI module before you can use the rad 3 shield.

That's not possible.
To get to liberty, you HAVE to have it...

You can get one much earlier if you saved the AI from Reagan's battlewagon and installed him in your vehicle, otherwise you get it in quest chain after you meet Angela.
