If that doesn't help then yes, sometimes they can phase through the barrier though it should be very rare.
Had this happen with a single Aardvark baby. It walked and swam half a mile across an enclosure over and over again every time I reset it just to wall phase through a specific point. The escape heat map showed nothing so I was very confused.
Finally I deleted that section of the barrier. Imagine my surprise to see a small speck of ground hovering in midair. It was debris left behind in mid-air and so small that it was actually fully inside the wall piece's visuals. However, the baby aardvark's movement code was seeing that spec of dirt as traversible terrain and phasing into the wall and then out the other side. Meanwhile the adult aardvarks and the other animals species I had sharing the habitat with them never even attempted the escape because they were so big that the dirt spec did not register for them as being traversible. As soon as I pushed away that dirt spec and then reinstalled the barrier between the exact same posts the baby animal stopped escaping.
Moral of the story... if you have a single escape artist in a habitat full of other animals that are not attempting escape then watch where they are escaping at and delete that section of the barrier just in case and clear out any odd looking terrain there then reset the barrier in place. Chances are this will fix your escapes issue.
Of course this will not help you if the issue is degraded barriers or if you left truly traversible ways of bypassing the barriers.
