Help with 100% map exploration

Guys need an extra set of eyes on my maps, I can't for the life of me see which place am I missing :(

So going by the map image I just posted...
-It looks like you're missing the secret room under the last A in "Live Ex Machina".
-Also, to the left of where it says "Garden of Silence", the secret room you access by breaking the floor. I remember for sure that's where you pick up the Santa Hat
-Directly below where you access the 8-Bit Nightmare secret in Hall of Termination, where the Teleport and Save rooms are one above the other, is another secret room
-Last looks like in the Underground Secret Labratory, right above the U.
