How to activate the Nightmare mode?

My question is simple and yet I haven't found an answer here, and I haven't been able to do it in the game.

I finished the game in Easy, Hard and I got the 100% in all categories, and all the achievements. However, the mode is still greyed out and does not allow me to choose it.

You can make a new save using the name NIGHTMARE.

Nightmare difficulty don't have level up system.

Ok so i need my save.

Update : I have restart Steam and the Game and miracle, i can choose Nightmare now... idk why i cant do this before...

I legitimately unlocked Nightmare mode by rushing the bad ending in Hard Mode.
