Also if you have more than one core the game might become confused (there were no multicore systems back in the day the game was made). The multicore thing you can fix by going into the Task manager and setting the Affinity to one of the cores, but that's messy so why not have the new .exe that forces that.
The new .exe is also playing nice with DEP, if you have that enabled.
Lastly the game is prone to crashing from time to time, the new .exe is far more stable in that regard.
Now, I'm not the maker of this .exe , but I am a community member and a modder of this game, and I feel obligated to point out the hard work that Kentie has put into making his .exe.
