Looks to me like a regular app exit and not some sort of error going by: Log: appRequestExit(0)
That's what it looks like when closing the game via the main menu or by the console command.
I see that you are using the original D3D renderer. I'd suggest moving to D3D9.

The reason I ask is because in another game, the mouse cursor would be over one button, but it was actually selecting another due to the resolution not being changed. Some of older games are buggy like this.
changing the resolution dosen't stop it crashing to the text box.
