Edit: It happens in fullscreen mode as well.
Then follow the modern set-up guide on steam.
Also I don't suggest following that guide, just do a fresh install, grab the latest kenties launcher , grab either the dx10, 9 or opengl enhanced renderer, I prefer the enhanced opengl renderer for the best performance and quality(a quick Google Search for "deus ex opengl" should hook ya up), and your good to go... I haven't found anything wrong with the game with this setup.... when you start adding those post processing effects and all of that, your bound to have problems somewhere down the line, it's best to just enjoy the game as it stands in all its glory
I fixed it making sure "Use single CPU" was OFF in Kentie's Configuration, now it works flawlessly.
I'm pretty sure that's the main culprit, but just in case these are all of my settings:
I have Raw Input and Widescreen cinematic subtitle fix both on, with custom GUI scale on and set to 1x.
I render with OpenGL Support with 144 FPS limit, Borderless fullscreen window viewport.
Audio latency is set to 60 with DirectSound enabled.
Single CPU off, 32-bit color and Detail Textures on (though they don't work), Automatic FOV.
