All endings in 1 save file
Can I get all endings with 1 save file?

you can get all 4 endings with one character in 3 playthroughs.

You need two playthroughs IIRC. Complete everything needed for the perfect ending EXCEPT don't pick up Io's final blood code of the ground (picking it up is the requirement, not viewing the memory so do not pick it up), finish game and get that ending. Come back to that code and pick it up and beat game for best ending. Then on the other playthrough get the bad ending. It will save you a run. This works because the final boss respawns after you beat it and credits roll. So you can repeatedly get the final endings depending on what you got unlocked, hence above trick, but yeah all on a single character with enough playthroughs as above poster indicated.

Right, this post is correct I had forgotten about the neutral ending and mixed it up with the get everything (rewards/gifts/codes/items/etc.) that is tangible which only needs two playthroughs.

You would need three playthroughs minimal, backing up save data and doing it from partway near end with neutral will not work apparently since it all must be on a single save to count for achievements, to get all the achievements. Achievements aside the endings, themselves, are purely story and don't matter for any reward so you only need two runs and really the endings kind of lol... esp that meme true ending (does it even legally qualify as a different ending? lol... anyways...). If you are doing it for achievements you can opt at NG+ start to go back into NG rather than NG+ to keep difficulty down for a quick run through. If you are just doing it to see the other endings, YouTube is your friend tbh... nothing worth replaying unless you absolutely just ridiculously love this game.

I got the neutral ending, but what if I just do 3 playthroughs for all of those endings. Does it work as well?

Yup, just get either the two best endings (remember what I said about picking up Io's final vestige... don't pick it up and beat boss then after credits go pick it up and beat boss again and as long as you met all the rest of the requirements this will be the deciding factor between which of the two best endings you get) in one run and the worst ending in the other. This will get you all endings and allow you to get all missing stuff with your next two runs since you already got the neutral ending.

Order doesn't matter so I'd pick the one that has the gifts you still need most and do that one (tho if you want whats her name Jack's GF at your base iirc that requires either of the two best endings due to saving her, tho even if she dies or you go into NG/NG+ she can still be used as a partner in combat).

all vestiges can be picked up without ruining the good ending playthrough, just leave a couple of Io's memories unrestored (this is the deciding factor) until you beat the final boss, then after credits rolled restore the rest of Io's memories and beat the final boss again earning the best ending.

Its actually picking it up that is the deciding factor, not restoring it. I had been concerned about which mattered, too, when I got to that point and looked online. Most opinions were picking it up was the trigger point but some conflicted, only a few, stating it was restoring it. I decided to test it by not picking it up and beating the game for the second best ending. Then I picked it up and killed the final boss again without actually restoring it and I got the best ending so it confirmed the matter (I was quite glad I didn't risk picking it up cause I nearly decided to). Also, for anyone who still is on the fence not picking it up doesn't set you back at all. You don't lose any progress and can just teleport to a mistle near it and run over and grab and then warp to final boss so it isn't worth the risk if you are unsure which answer is right as it isn't like you are repeating an hour or three and several fights or dungeons to do this.

Why I got neutral ending : I forgot to restore Nicola's memory and I already did collect and restore all of Eos vestiges. Will it spawn in my next playthough?

No idea why it was like this in your playthrough. Maybe you accidentally did restore her memories? I did the good ending + "true" ending recently and picked up all vestiges before the good ending. Only restoring all of Ios memories decides whether you get the "true" ending or not.
